Financial Aid Forms

Please be sure to read and answer every question on the forms before submission. If a question is left blank it may delay you from receiving financial aid.


The completed forms need be turned into the IRIS office.  This can be accomplished several ways:

  • The forms may be returned in person
  • Emailed to with the student's ID number (700#) on each page, or
  • Faxed to 970-248-1191 with the student's ID number (700#) on each page

Summer 2019 Supplemental Aid Application

This form must be completed and submitted to IRIS in order for you to be considered for summer financial aid.  Submit this form after you have registered for classes.

Eligibility for Summer 2019 Aid:

  • Must have a 2018-19 FAFSA submitted and on file with the Financial Aid office by June 30, 2019, or the last day of your classes, whichever is earlier.
  • Must be enrolled in a minimum of six (6) credits to receive funding through state and most Federal sources (Minimum of one credit hour for Pell Grant, if eligibility exists).
  • Any change in credit hours from those stated on this application may result in a change/elimination of funding for summer.
  • Funding must be used in accordance with Federal guidelines regarding the receipt of Federal funds; lifetime eligibility usage limits apply.

What happens if I do not attend?

  • Any student who receives financial aid and does not attend class(es), will be required to repay all aid before re-enrolling at Colorado Mesa University.

What happens if I withdraw or receive all F or I grades for summer?

  • Any student who receives financial aid and withdraws from all summer 2019 classes, or receives all F or I grades, will automatically become ineligible for financial aid for future semesters and repayment of aid will be due immediately.
