The U.S. Department of Education has request that you, the student, submit proof of your identity to the Financial Aid Office. Please print the following form and return it with a valid unexpired government-issued photo ID to the CMU IRIS desk.

If you have had a prior appeal with Colorado Mesa University approved for independent status new federal guidelines allow our office to renew that status annually without having to submit a new appeal when you submit a future FAFSA.
