Dependent Tuition Waiver Form - An employee's dependant is one who satisfies the general eligibility requirements for purposes of eligibility for health benefits under Colorado Mesa University health insurance.

Tuition Waiver Form - The tuition waiver program is designed to assist employees in expanding their education through courses at Colorado Mesa University .

Payroll Direct Deposit Form - This will allow the college to directly deposit your pay check into a designated checking or saving account. Please find this on-line form in your MAVzone account. This form will be found under your Work Life tab in your Student Payroll Channel.

 

 



General

Address Change - This form will update your current address for the  departments selected on the form.  You can change your

address on-line through your MavZone account under your Student Academic tab under the Student Quick Links channel.

1098-T - Students can review their information via the web by visiting ECSI and following the login instructions to access the tax form. 

Third Party Billing - Some employers/businesses pay for tuition, fees and books for students.  This needs to be set up in advance with our A/R Office.  The authorization can be saved and e-mailed or printed and faxed to us at 970.248.1136.

College Opportunity Fund - The College Opportunity Fund (COF), created by the Colorado Legislature, provides a stipend to eligible under-graduate students. The stipend pays a portion of your total in-state tuition when you attend a Colorado public institution or a participating private institution.
